Care Plus

The aim of Care Plus is to provide enhanced services to 50% patients with complex health care needs enrolled with Te Tai Tokerau PHO GP providers who meet Care Plus criteria.

The Care Plus programme is being implemented in varying degrees in the majority of practices Patient management resources have been developed and provided to practices, with ongoing support and education provided by a dedicated support nurse,

There are two nursing resource people employed by Te Tai Tokerau PHO to support practices achieve the best results for patients eligible for Care Plus.  One nurse focuses on continuous quality improvement and the second provides support to practices to better meet the needs of Māori patients.

PMS systems have been configured to ensure ease of patient management and invoicing.

Patients enrolled in Care Plus programme receive subsidized enhanced quarterly visits to providers.

1213 patients from an eligible 2776 across the PHO have been enrolled in Care Plus to date.

A Flinders model Motivational interviewing training package was provided for nurses in November 2007, 18 nurses from various organisations attended the full day workshop
