Management of Cardiovascular Disease

Objectives
To Implement a series of actions across the care continuum, including a major awareness campaign, to reduce the impact of cardiovascular disease in the enrolled population particularly those currently under-serviced

Project
This initiative resources and incentivises providers initiate activities that build on awareness gained through the One Heart Many Lives campaign.

i)  resources nursing time for outreach screening, which is accompanied by a Warrant of Fitness card, The WOF indicates the following based on a traffic light for health:

  • RED light -a risk factor of 20% or more indicated the need for a comprehensive heart assessment by the participant's doctor or nurse within the next 2-4 weeks.
  • ORANGE light -a risk factor between 10- 20% indicated the need for a comprehensive heart assessment by the participant's doctor or nurse within the next 2-4 months.
  • GREEN light - a risk factor of less than 10% indicated the need for a comprehensive heart assessment by the participant's doctor or nurse within the next 12 months.

ii) Tane Māori roopu support with ongoing meetings and educational hui,
iii) the Bro files consisting of regular media initiatives and feedback
iv) A range of HEHA activities to support the key message of improved cardiac health

  • Numbers indicate that Māori men do want to be healthy
  • There is a need to continue this health promotion to reduce health disparity in this area of health
  • Proven need for ongoing evaluations from this initiative

A total of 2495 eligible patients have been screened using PREDICT; quarterly incentive payments have been made to practices with a formula based on ethnicity and deprivation.
