This project aims at Improving health and wellbeing outcomes for Māori patients with coronary heart disease
Project
The objectives of the programme are to:
- Increase the number of Māori who are referred to, participate and complete a phase two cardiac rehabilitation
- Increase the participation of whanau in cardiac rehabilitation programmes
- Increase opportunities for whanau to be screened for cardiovascular disease
- Increase community awareness of the impact of coronary heart disease on the Māori population, including the causal associations, heart disease prevention strategies, early identification of risk factors, best practice treatment for coronary heart diseases and increased confidence in seeking best practice treatment.
