Objectives
The service targets clients who have been on a sickness of unemployment benefit for twelve months or less with the aim of supporting clients back into employment by addressing mental and physical health needs. 

Ministry of Social Development Project
The pilot project will be rolled out in Northland through Manaia PHO, TTTPHO and Hokianga.  Te Tai Tokerau PHO employs a  Mental Health Practitioner on a 0.5FTE basis  to manage 177 clients over 18 months.  The Practitioner assists clients to  be reintegrated into workforce by addressing health problems through fully resourced packages of care, available according to individual client needs.
